Avenues Short and Long-Term Disability Plan
The Avenues Short and Long-term disability plan is currently underwritten by Genworth Financial. Any employee who is classified to work a regular minimum of 32 hours per week and has completed 90 calendar days as an Avenues' employee, will be eligible for coverage under this plan.Enrollment will begin on the 1st day of month following your 90 day waiting period. Check with your immediate supervisor if you have any questions about your eligibility.
There is no cost to the employee for this benefit. This policy covers eligible Avenues' employees only.
Subject to medical review, beginning 30 days after you have been confirmed as disabled(as defined under the plan), this policy will pay you $250 per week for a maximum of 22 weeks.
Should the disability continue, beginning 180 days after you have been confirmed as disabled(as defined under the plan), this policy will pay you up to 60% of your salary at the time that the disability was incurred, up to a maximum of $3,000 per month, up to the age of 65. Reduced benefits will apply if the disability begins after the age of 60.
Please remember that unless you have available vacation, sick or personal time off, no other compensation will be forthcoming from Avenues.
Please contact your immediate supervisor if you become disabled and are unable to return to work for any length of time.
These plans do not provide coverage due to any occupational illness or injury. This would be covered under the Workmen's Compensation benefit plan.
