Dental/Vision Plan

Avenues is pleased to offer a dental hmo and vision discount plan to its employees.  This plan is available to employees with 90+ days of service, regularly scheduled to work 32 hours or more per week.  Premiums are 100% paid by you through a payroll deduction.  Avenues will not pay anything toward this benefit.  Check the premium deduction webpage for additional information.

A dentist must be chosen from the Assurant/Fortis Pinnacle, Legend Series Dental HMO Directory.  This is available online at the Dentists search page .  Make sure the page shows the Heritage series; scroll down, put in your information and click submit.  Not everyone’s dentist will be listed, but this is what we have to accept with an HMO. 

You will need to print out, complete and return the Assurant Dental application to receive this benefit.  Make sure to include the dentist's name and identification number.  Payroll deduction will begin during the first full month that your application is approved for coverage.  

The vision plan is a discount program only.  It offers 20% off at participating eye doctors for exams and glasses, 15% off on contact lenses and possible discounts on laser surgery.  Go to www.vsp.com or call 1-800-877-7195 for information.  You will automatically receive the vision program with the dental hmo.  Vision is not available as a separate benefit.
