Avenues Life and Accidental Death Insurance

Life insurance and accidental death coverage are provided to employees who have completed 90 days of employment and who work a regularly scheduled minimum of 32 hours per week.

Not all employees will receive these benefits. If in doubt, please verify your status with your immediate supervisor.

There is no cost to the employee for this benefit. This policy covers eligible Avenues' employees only.

This benefit will pay your designated beneficiary $15,000 in the event of your death.

All coverage amounts are reduced for employees past age 65. See your individual plan for details.

These plans do not provide coverage due to any occupational illness or injury. This would be covered under the
Workmen's Compensation benefit plan.
