Avenues Health InsuranceDuring calendar year 2008, the Avenues Health Plan will be underwritten by Blue Cross Blue Shield of Illinois. Any employee who is classified to regularly work a minimum of 32 hours per week and has completed 90 calendar days as an Avenues' employee, will be eligible for coverage under this plan.
Enrollment will begin on the first day of the month immediately following your 90th day of employment. Check with your immediate supervisor if you have any questions about your eligibility.
Currently, 100% of the premium cost is paid for employee only basic HMO coverage. Higher HMO, PPO, Family and dependent coverages are optionally available through payroll deduction at the employee's expense. Avenues Employees have a choice of the BCBS Blue Advantage HMO or BCBS PPO.
